visit is to begin to discover WHY you are suffering or struggling with your health concern.
The ultimate goal being to provide you with an explanation as to why you have your health
problem. While a name for you condition might be appropriate, it is far from successful to
simply give you a new, another identity…we want answers.
This inquiry may lead to recommendations for further testing or diagnostics.
*To aid in our providing recommendations and a solution, please complete the
following forms...they are an integral part of your diagnostic process...which means
finally answering WHY you have your problem, and they contribute to the design of
the treatment protocol (the solution)...SO PLEASE BE COMPLETE AND DETAILED.
